Formosa Training for the Point Comfort Site has changed. Contractors entering the site must have specific training per the Formosa Training Matrix issued by Formosa at the time the contract was issued to perform the work. If you are unsure what training modules your employees need to be compliant for the work to be done at the FPC facility, Please contact Steven Schultz @

Once you have determined what Formosa training modules are required for compliance, please download a

Contractors' Safety Council Registration Form

and a

Formosa Plastics Training Registration Form

from the links above in blue. There must be one of each form for each trainee that will arrive. The training forms are to be faxed over by 4:00pm the day before training is to be administered here in the council. Training will be administered on a first come / first served basis.

The Contractors' Safety Council will NOT assume what training is required by Formosa for compliance!

It is the contractors responsibility to register what is needed or the trainee will NOT be trained!
